| Applicant and sponsor tour facilities at 3430, 3480 and 3486 Prescott Street, and meet with Executive Director and/or Program Coordinator, to discuss admission procedure
, and obtain application forms (Medical, Release of Information Waiver, and Questionnaire). Admission to the services and programs is limited to individuals who are intellectually challenged, twenty-one (21) years of age and over, and reside in the former City of Halifax. |
| Complete application forms and return. Applicant's name is placed on a list of applicants for processing. |
| Applicant is given a date for 1-2 week assessment placement. |
| Program Committee is informed of the applicant's assessment progress, and recommendations for consideration. |
| Applicant is advised in writing of their status. |
| If approved for probationary status, the applicant begins a three-month probationary placement when a position becomes available in the required program. The following summarizes the program options and training available to applicants. |
